When the controller gives me an altimeter he says "Altimeter 1023" or some millibar value even though I'm flying in the US. I played with the Use QNH Misc Option but that doesn't give me the correct response. Am I missing something with the settings.

Select the Options #1 screen, and then (at the bottom) the Trans Alt & Alt Baro Calls screen. Select the regions where you want baro calls in millibars and untick (de-select) the regions where you do not.
