PF3 has multiple routing algorithms. First it tries a fast one, and when this does not return a result, it tries the other one. At this point you hear the second "stand by" call. I had situations where this second algorithm chokes on a complicated AFCAD and needs more than 5 minutes to come up with long results like:
J-Bird 1608 Roger. Taxi to holding point, runway 6 using taxiway Golf 2 Golf Golf 3 Golf Hotel 14 Golf Golf 5 Echo 1 Sierra 7 Sierra 6 Sierra 5 Bravo Sierra 4 Bravo Sierra 3 Bravo Sierra 2 Romeo Sierra 1 Romeo

During this time ATC is so busy that it ignores you (and everyone else). Just be patient, when PF3 is finished, it will talk to you.
Things that helped me preventing such situations:
- using a different parking spot
- using a different AFCAD

Additionally, when you disconnect PF3 in this situation, it does not clear the hotkeys registered with FSUIPC, and as the number of allowed hotkeys is limited (I think 56 for all add-ons combined), you will quickly run out of slots.

Two "Standbys" happen a lot when a taxiway connects to the centre part of a runway i.e. "not" at the far end. If as Thomas says your route to that point is complex or broken (as is often the case) then than can cause PF3 some problems. A simple example is LIMZ. It has a single runway 03-21 and a single taxiway "alpha" which joins the centre of the runway which has two turning points at either end. If the take-off is from 03, PF3 says "standby" twice and then "taxi via alpha". But if the take-off is 21 I get only one "standby" and "Taxi via taxiway." (note: this was also present in PFE). Which is a bit odd as in this case there is only one taxiway. So always have a close look at the afcad file and as Thomas says try a different parking spot or a better designed afcad. Having said that I have never had to wait five minutes. Just a few seconds between the "Standbys".

Yes he said "standby" twice...I guess he really meant it!...And it is a very complex afcad with more AI than ive ever seen...But I didnt hear ATC talking to any one else, and no one was coming or going?

I had this happen yesterday at a Orbx KPDX, I restarted P3D and PF3 and same issue, two quick standby's by ground when asking for taxi instructions, then it locked up. Finally restarted again at a different parking spot, and it worked fine.

There will be only seconds between first and second standby, but up to several minutes between the second standby and Ground giving you the route.
PF3 will not talk to other aircraft, react to hotkeys or do anything else during this time. You need to be patient here.
The amount of AI does not matter.
(my 5 minutes was with default FSX EHAM AFCAD - five runways and many taxiways)

Guess I didn't wait long enough then.

What parking spot were you at Thomas and what runway were you supposed to taxi to?

I'll have a go tomorrow and see how many weeks PF3 takes to reply :-)
