Can't De-activate

Sorry to bug u about this again but I can't de-activate PF3. When I go to Options#2 & select de-activate, a window pops up requesting my activation code. I enter it and then a window comes up with empty boxes, devoid of any text/instructions with 'Activate' and 'Cancel' buttons at the bottom.

I have to switch pc's around cuz my wife's pc died. Appreciate any assist.
