In the past when I have declined the visual, the response was a verbal new runway instruction followed by a new heading. I now get no response at all

Nothings changed in this feature but if PF3 can't find a suitable runway when requested your request will be declined. You could always try using the C\Z=S\W hotkey and choose the runway you want, as it's a little more forgiving.
