I say this because whenever I fly to an airport on an island: Singapore, Iceland, Sicily, Sardinia for example they quote air pressure in inches instead of millibars and have transition levels of 18,000ft.

How can I ensure these islands are included within the ICAO rules?

Yes David I have and that's why I am curious as to why for example Singapore quotes inches instead of millibars and has a transition level of 18000 as do others like Iceland for example.

It would seem that somehow major islands are excluded from their associated land masses and therefore come under US control!!

Another point about American pilots (voices) - American pilot's licences from PPL up to ATPL are not valid in Uk and Europe. In order for an American pilot to fly into say UK airspace his airline has to have passed an accreditation to the CAA that the said pilot has undergone retraining and has the neccessary upgraded rating. My point being that often when I hear on PFE an American pilot whilst flying in Europe they still say e.g. 119 point 55 instead of 119 decimal 55.

If all the other pilots can remember to say decimal why can't the Americans!!!??

Well I would need to see a log to determine if and where there is a problem but my 'guess' is the regional settings are defines by controller voice set accent rather than the area itself, if so I will add that to my list of things to rectify.

As for US voices saying 'point' rather than 'decimal' again that is because the variations are determined by accent and not area. We can't get everything right and besides if I attempted to model PFE on real world ATC you would hear some controllers and pilots chatting in a languages other than English.... which, according to my pilot chums, happens quite frequently in Europe.
