Part of pilot training is to maintain a "listening watch" as they say. This means that you are constantly listening out for your call sign at all phases of the flight. And, you are expected to reply immediately with the correct response. "Say again" is not allowed in the RT exam. In fact if you fail the RT exam that's pretty much the end of your flying career! If you fluff the "Mayday" call that's an automatic fail and the exam stops there and then.
BTW "Mayday" comes from French "m'aider".
