Any way to start PFE smoothly with FSX started at a runway position rather than a gate?

Yes, just position yourself at the runway then ask for clearance... you will be handed to ground control then tower

Without knowing which is the active runway at startup PFE usually directs me to taxi to a different runway than I am at. I am looking for a way to get clearence for whatever runway I am currently on.

Okay, after you receive taxi clearance, if you require a different runway use hotket #1 to deactivate TGS then C\S\W to request another runway

I usually just go to "weather" and set the wind direction...works for me...
